Youth Transition Services

Our Youth Transition Services program is designed to help youth with disabilities transition into adulthood.  We understand many of the challenges young people face and can assist in the decision-making process as individuals move toward greater independence in their communities.

This program provides all of the core services provided to all consumers with particular emphasis on areas relevant to each youth.  Areas to address may include the following:

  • Applying for funding and following up with benefits claims
  • Independent living skills training
  • Finding an affordable place to live
  • Locating personal and homemaker care
  • Looking for a job
  • Help with budgeting
  • Making use of assistive technology and accessibility resources
  • Learning to use public transportation
  • Continuing secondary education or trade training
  • Finding opportunities for social interaction
